In-universe, we agree that opium are laudanum are indeed the drugs of the time and can easily found throughout Andovale. It can be found at most drug stores as a sort of pain relief pill. We do worry that this dependency could affect his time in game, as in making it difficult for him to form bonds with others whilst being so deep in addiction, but if that's something you're comfortable playing with and working through, we have no problem with choosing this particular canon point.
As for doctors, yes, Victor should be able to obtain a business license and practice his medicine without issue. Depending on the type of medicine he does, locals may be wary. This is a time of seawater, leeching, and other terrible medical practices. If Victor shows up doing real, actual medicine, people might find him strange. What is this new-age witchcraft??
